New Passport (Form DS-11)

You must use this form if you meet one of the following criteria:

  • You are applying for your first U.S. passport.
  • You are 16 or older and your previous U.S. passport was issued when you were under age 16.
  • Your previous U.S. passport was issued more than 15 years ago.

You must use this form if your passport meets all of the following criteria:

  • Is not damaged and can be submitted with your application.
  • Was issued when you were age 16 or older.
  • Was issued within the last 15 years.
  • Was issued in your current name or you can legally document your name change.
Lost, Stolen or Damaged Passport (DS-11 and DS-64 Form)
