You need to contact the National Passport Center and ask them what to do. Here&#039;s their number: 1-877-487-2778. Also, I&#039;d recommend contacting your local member of Congress and requesting assistance, especially if you aren&#039;t able to get a resolution from the Department of State.</description>

Sally Mae is mentally handicapped and unable to provide any information about her background. Her mother&#039;s name was Annie Mae Moody and her Father&#039;s name was Harmon Moody. She has a sister named Annie Ruth, age 68, who has a birth certificate. She also has another sister, Francis Louise Moody, aged 71. All were born in Fairmont, N.C. Francis knows when her younger sister was born, but she&#039;s physically unable to sign because of her health.</description>
